Our privacy practices are outlined in the Consent for Services document signed by all clients prior to receiving services at our agency. As outlined in this document, Learning Dynamics reserves the right to change the terms of our Consent for Services document and our privacy practices. Physical copies of current practices are maintained and available in our Central (Bakersfield) and Southern (Woodland Hills) California offices. Upon a client’s request, we will provide a copy of the revision. Please remember, the new (current) notice is effective for any health information that we already have about a client, as well as any information we receive in the future.

Of importance, and consistent with the California Health and Safety Code 123130, should a client wish to inspect or receive copies of their records with our agency, or wish for us to provide access to a third party; they understand and agree to be provided with a summary, rather than receiving access to the entire record. There is a fee for this service. If a client feels the information in their file is incorrect or incomplete, they may prepare a correcting statement to be included in their records as an addendum.

Feedback on Services and Filing Complaints

Learning Dynamics welcomes feedback and receives and responds to complaints regarding our services, including the practice of psychotherapy, by any staff or practitioner providing services at our agency. To provide positive or constructive feedback, or to file a complaint (e.g., dissatisfaction with services or staff), you can contact our Executive Director, Nicole Brown or our Associate Director, Lizeth Lopez  via phone at (310) 855-3276, via fax at (310) 393-9893, via email at info@learningdynamicsinc.org, or you may direct written correspondence to 22837 Ventura Blvd., Suite 200, Woodland Hills, CA 91364. If a client believes their privacy rights have been violated, they must file a complaint with Learning Dynamics, Inc. in writing.

In addition, if the practitioner is registered with the California Board of Behavioral Sciences, the California Board of Psychology, and/or another state’s licensing board, these agencies also receive and respond to complaints regarding the practice of psychology by individuals licensed and registered with their board. If you have a complaint and are unsure if your practitioner is licensed or registered, please ask your provider or use one of the links provided above to verify their license. Here is a list of state licensing boards and their contact information for the states our practitioners are licensed in. A client’s services will not be affected in any way if they file a complaint.
