Learning Dynamics Notice of Privacy Practices
Our privacy practices are outlined in the Consent for Services document signed by all clients prior to receiving services at our agency. As outlined in this document, Learning Dynamics reserves the right to change the terms of our Consent for Services document and our privacy practices. Physical copies of current practices are maintained and available in our Central (Bakersfield) and Southern (Woodland Hills) California offices. Upon a client’s request, we will provide a copy of the revision. Please remember, the new (current) notice is effective for any health information that we already have about a client, as well as any information we receive in the future.
Of importance, and consistent with the California Health and Safety Code 123130, should a client wish to inspect or receive copies of their records with our agency, or wish for us to provide access to a third party; they understand and agree to be provided with a summary, rather than receiving access to the entire record. There is a fee for this service. If a client feels the information in their file is incorrect or incomplete, they may prepare a correcting statement to be included in their records as an addendum.
If a client believes their privacy rights have been violated, they may file a complaint with Learning Dynamics, Inc. in writing. All written communication with Learning Dynamics should be directed to 22837 Ventura Blvd., Suite 200, Woodland Hills, California 91364. A client’s services will not be affected in any way if they file a complaint.
